Section 4(d)(1) of the Assistive Technology Act of 1998, as amended (AT Act)requires that states submit an application to the Rehabilitation Services Administration (RSA) to receive a grant. RSA calls the application a State Plan for Assistive Technology (State Plan for AT or State Plan). This document provides the instructions for completing the State Plan that covers the activities of Statewide AT Programs for Federal fiscal years 2012 (October 1, 2011 to September 30, 2012), 2013 (October 1, 2012 to September 30, 2013), and 2014 (October 1, 2013 to September 30, 2014).
